NOW, THEREFORE, in consideration of the mutual promises contained herein, the parties agree as follows:

  1. Acknowledgment of Violation
    The Employee acknowledges the violation of the Company’s drug and alcohol policy and accepts responsibility for their actions.
  2. Commitment to Compliance
    The Employee agrees to comply with the following conditions as part of this Agreement:
    • Immediate cessation of all drug and/or alcohol use that violates Company policy.
    • Participation in a Company-approved substance abuse program or counseling as directed by the Company.
    • Submission to random drug and/or alcohol testing as determined by the Company for a period of [specify period, e.g., one year].
    • Attendance and completion of any required follow-up meetings or sessions as directed by the substance abuse program or counselor.
    • Compliance with all Company policies and procedures, including the drug and alcohol policy.
  3. Drug Testing
    • The Employee agrees to submit to random drug and/or alcohol testing at the discretion of the Company.
    • Testing will be conducted in accordance with the Company's drug testing policy.
    • A positive test result or refusal to submit to testing will be considered a violation of this Agreement.
  4. Duration of AgreementThis Agreement will remain in effect for a period of [specify period, e.g., one year] from the date of execution.
  5. Consequences of Non-Compliance
    The Employee understands and agrees that any violation of this Agreement or any further violation of the Company’s policies will result in immediate termination of employment.
