Home Health Aides job description

Job Brief

We are looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patient’s homes in accordance with an established care plan.

As a Home Health Aide, you will work with patients who have a variety of medical conditions and disabilities. You will be responsible for providing care that helps them maintain their independence and dignity while living at home.

You will work closely with other healthcare professionals such as registered nurses and therapists to ensure that patients receive the best possible care. You will also be responsible for monitoring patients’ vital signs and recording their condition.

You will escort patients to appointments and social events and maintain records of patient care, condition, progress, and problems in order to report and discuss observations with a supervisor or case manager. You will adhere to professional standards, follow policies and procedures, and comply with federal, state, and local requirements.

Responsibilities

• Assist patients with activities of daily living such as bathing, grooming, dressing, and toileting.
• Perform light housekeeping duties such as laundry, washing dishes, and vacuuming.
• Plan and prepare meals according to dietary requirements.
• Provide medication reminders.
• Escort patients to appointments and social events.
• Maintain records of patient care, condition, progress, and problems in order to report and discuss observations with a supervisor or case manager.
• Adhere to professional standards, follow policies and procedures, and comply with federal, state, and local requirements.

Requirements

• Proven working experience as a home health aide or personal care aide
• Ability to monitor vital signs and record patient condition
• Compassionate and empathetic with good communication skills
• Ability to work independently with minimal supervision
• Physical endurance to perform duties such as lifting and transferring patients
• High school diploma or equivalent
